Direct Deposit Sign Up Forms

For social security, supplemental security income, civil non-military federal retirement and railroad retirement payments use FMS 1200. For all other non-vendor federal payments, use SF 1199A. (Mail to the appropriate paying agency, or contact your financial institution.)
